You can file a claim one of the following ways:
Access the Colonial life portal to file a claim online.
For the Health/Wellness Screening claim form, you can file by fax or by mail.
Lastly, you can file a claim by phone. Call the Colonial Life Services Center at 1-800-325-4368. When you file by phone, you will need the following information: type of test and the provider’s name, address, and phone number.
