As you transition to your new benefits, please keep in mind:

Open Enrollment Starts May 15th for EMS Personnel

Medical:
- Basic Plan and Choice Plan information can be reviewed here: Basic | Choice
- TelaDoc registration and information can be accessed here: TelaDoc
Dental / Vision:
- Be sure to complete any current treatment before joining your new plan. Review continuing care policies with MetLife before your plan changes here
- You will not be receiving a dental or vision card
Pharmacy:
- Fill your medications two weeks prior to your current insurance cancellation
- Use Kroger Pharmacy to reduce your headaches and the Trust's cost
- Call Matt Otey (Healthcare Trustee) 817-919-6994 if you have issues with your prescriptions
- You will NOT get a separate Pharmacy card
- If you ARE currently take a high cost or specialty drug - contact Matt Otey 817-919-6994
